How do I get started in the 20/20 Lifestyles Program?
All participants are required to attend our free Introductory Seminar. The seminar is by reservation only. You may view the seminar schedule here. Please call or visit the 20/20 Front Desk to reserve your space at (425) 861-6258.
Will my insurance pay for the program?
Currently we accept Premera Blue Cross Insurance Plans for Microsoft employees. If you or your spouse are not covered under Microsoft's benefits plan, you will be responsible for the cost of the program.
I attended the Introductory Seminar and received the email link, what is next?
Once you have completed and provided your online forms, you will need to make an appointment with your physician to obtain a referral, (and if Microsoft) your Premera Weight Management Recommendation Form.
How do I know if my 20/20 file is complete?
Please use the "Steps to Getting Started" checklist to verify that you have completed all the necessary forms. Then call the 20/20 Lifestyles Front Desk, (425) 861-6258, to confirm that all forms have been received and are complete. The Front Desk will then verify your requests for training days, times and location preferences.
My paperwork is complete. When can I get started?
As soon as your requested training time becomes available, you will receive a call from a 20/20 patient coordinator to schedule you into the program.
I am a Microsoft employee and have Premera Insurance. What will they pay for and what do I pay?
Premera will cover 80% of the costs of the program up to a maximum allowable amount of $6,000. You will be responsible for 20% of the cost of any length of program and $1,000 is required on the day of your initial Fitness Assessment. Financing options of remaining out of pocket costs are available.
Are financing options available?
Yes, clients may take advantage of several financing options. After an initial down payment of at least $1000, a client’s remaining balance may be paid throughout the program via monthly installments at nominal six percent rate. Clients may also elect to extend their payment plan for up to two years post program. Depending on the length of the payment period, the interest rate will vary from six to eight percent.
How do I know which program length is best for me?
Premera and 20/20 staff will recommend the appropriate program length for you based on your BMI, but you may choose a 12, 16 or 20 week program. If needed, you can decide later to modify your program length to upgrade to a longer program.
If you are a Microsoft employee, Premera requires you to also participate in Phase 2 or the 12 week Transitional Program. This is included in the program cost.
